HIPAA Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW LIMITED HEALTH-RELATED INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

1. Who We Are

Southern Nevada Health Pass ("SNHP," "we," "us," or "our") is an independent benefits-education organization. We are not a healthcare provider, health plan, healthcare clearinghouse, insurance company, or government agency. We help members of Anthem, Molina, SilverSummit, CareSource, Health Plan of Nevada, and Medicare discover and activate the free supplemental benefits already included with their plans.

Although we are not a HIPAA "covered entity," we voluntarily apply HIPAA-aligned safeguards to the limited health-related information you choose to share with us, and we have signed Business Associate Agreements (BAAs) with our backend and email-delivery vendors.

2. Information We Collect

When you use our lead form, we collect only what is needed to identify the right benefits:

  • First and last name
  • Phone number and email address
  • Date of birth
  • Insurance carrier and (optionally) member ID number
  • ZIP code

We do not collect medical records, diagnoses, treatment history, prescriptions, lab results, Social Security numbers, or financial account information. We do not knowingly access Protected Health Information (PHI) from your insurance carrier.

3. How We Use & Disclose Your Information

We use the limited information you provide only to:

  • Match you to the free supplemental benefits available through your specific insurance plan;
  • Email you the benefits guide that corresponds to your carrier;
  • Have a benefits navigator call/text you to help you activate those benefits;
  • Comply with applicable law, subpoena, or other legal process.

We do not sell, rent, or trade your information. We do not share it for advertising or marketing by third parties. We do not disclose it to your insurance carrier without your explicit request.

4. Your Authorization

By submitting our lead form and checking the authorization box, you authorize SNHP to use the limited health-related information described above for the purposes listed in Section 3. You may revoke this authorization at any time by emailing privacy@southernnevadahealthpass.com. Revocation does not apply to information already used or disclosed in reliance on your prior authorization.

5. Your Rights

You have the right to:

  • Access a copy of the information we hold about you;
  • Correct information that is inaccurate or incomplete;
  • Delete your information from our systems (subject to legal retention requirements);
  • Restrict certain uses or disclosures;
  • Opt out of calls, texts, and emails at any time (reply STOP, click unsubscribe, or contact us);
  • Receive notification in the event of a breach of unsecured information that affects you.

To exercise any of these rights, contact our Privacy Officer using the information in Section 9.

6. How We Protect Your Information

  • All form submissions are transmitted over TLS 1.2+ encryption.
  • Information is stored in access-controlled databases with row-level security.
  • Vendor access is governed by signed Business Associate Agreements (BAAs).
  • We minimize information in logs, do not store medical records, and limit staff access on a need-to-know basis.
  • We retain information only as long as needed to provide the service or comply with law.

7. Breach Notification

If your information is compromised in a manner that creates a significant risk of harm, we will notify you without unreasonable delay and in accordance with applicable federal and Nevada state law (NRS 603A).

8. Changes to This Notice

We may update this Notice from time to time. The current version is always posted at /hipaa-notice with an updated effective date. Material changes will be communicated through the website.
