What you can actually earn

Anthem's Healthy Rewards program pays $10 to $50 per qualifying activity, in retailer gift cards — not cash.

Enrollment is required

You must be enrolled in Healthy Rewards before your visit, or within 30 days of the date of service. Members lose rewards most often by completing the visit first and enrolling later.

Rewards are capped, not repeatable all year

Each activity can be earned only a set number of times — once every 12, 24 or 36 months, or once per lifetime, depending on the activity. Most activities are also age- or sex-restricted. There is no version of this program where you earn a reward for every visit you make during the year.

What is not on Anthem's list

Dental exams and vision screenings are not in Anthem's Healthy Rewards schedule, even though they are covered services. Ask Anthem for the current reward schedule for your plan year before you count on a specific payment.
