Extra Help (the Low-Income Subsidy) lowers what you pay for Part D drugs. For 2026:

• Income: up to $23,940 a year for an individual, $32,460 for a married couple.
• Resources: up to $18,090 for an individual, $36,100 for a married couple.

The resource limits matter as much as the income ones. Savings and certain assets count, and people who pass the income test are often turned down on resources. Both tests apply — apply anyway if you are close, because the rules on what counts are narrower than most people assume.

What drugs actually cost under full Extra Help

Not $0 from day one. In 2026, under full Extra Help, you pay up to $5.10 for a generic and up to $12.65 for a brand-name drug until total drug costs reach $2,100, and $0 after that.

The out-of-pocket cap

The Part D out-of-pocket cap is $2,100 for 2026.

Deductible

The 2026 Part D deductible ceiling is $615. Some plans charge $0 — it varies by plan.

Premiums: be careful with "$0 plans"

$0 premiums are common in Medicare Advantage plans that include drug coverage. For stand-alone Part D plans it is a different picture: of the roughly 11 stand-alone options an average beneficiary can choose from, only about 2 are $0 premium, and the average premium is near $36 a month.

Moving and special enrollment

A move can create a Special Enrollment Period — but a move within the same county usually does not qualify. It generally has to take you outside your plan's service area. Check before you assume you can switch plans mid-year.
