HPN Gift Card Rewards: $15–$25 Preventive Care, $25–$100 Behavioral Health

The amounts

• $15 to $25 for adult and pediatric preventive care activities.
• $25 to $100 for behavioral health activities.
• $500 annual cap per family.

Nothing is sent automatically — you have to claim it

Earlier versions of this page said HPN would send the card "as a thank-you" if you "simply keep up with your recommended care schedule". That is not how it works. The steps are:

1. Sign in to the HPN app or the online member centre.
2. Go to Additional Benefits → Member Perks.
3. Submit the member information form.
4. Wait 6 to 8 weeks for the card.

If you never submit the form, you never get paid — no matter how much preventive care you have completed.

Behavioral health claim windows are strict

• 7 days for substance use disorder follow-up after the triggering event.
• 34 days for continuation of treatment.

Miss those windows and the reward does not pay, even though the care still helps you.

What qualifies, and what does not

• The adult list is a well-child visit at ages 18–21 plus specific screenings — not general "annual wellness visits" at any age.
• COVID vaccination is not on the reward list.
• The HPV dose reward is a Foot Locker gift card, not Walmart.

Important: HPN's Medicaid service area changed on 1 January 2026

As of 1 January 2026, Health Plan of Nevada's Medicaid service area is urban Clark County only. Members in Washoe County and rural Clark County moved to CareSource. If you live in one of those areas, the benefits on this page are probably no longer yours — check which plan you are in now before you rely on anything here.
