The reward

SilverSummit adds $30 to your My Health Pays card for completing an annual wellness visit with your primary care provider.

Who qualifies — read the age range carefully

There is an upper age limit of 64. Members 65 and older are not covered by this reward row.

On the lower bound, SilverSummit's own two sources disagree: the live rewards page says age 22, while the 2026 fact sheet says 21. So the range is 21 or 22 through 64, depending which document you read. If you are 21, book the visit anyway and ask SilverSummit to confirm before you count on the money.

How the My Health Pays card actually works

Rewards load onto a My Health Pays prepaid card. It is not a card you can use anywhere Visa debit is accepted — SilverSummit says so plainly, and this is where most members lose value.

Permitted uses: utilities, transportation, telecommunications (phone and internet), childcare, education, rent, and Walmart in-store.

Not permitted: grocers, gas stations and pharmacies are not on SilverSummit's approved list. The card also cannot be used for alcohol, tobacco or firearms.

Funds expire — this is how people lose money they earned

Your balance expires 90 days after your coverage ends, or 365 days after the reward was earned, whichever comes first. Medicaid coverage lapses at redetermination more often than for any other reason, so spend the balance rather than saving it.
